What paperwork will I receive after I purchase a policy?
  • After your purchase, you will receive a certificate of insurance. If you requested certificate holders or additional insureds, these will also be included. If you would like a copy of your master policy, please email us at [email protected].
How will the charge appear on my bill or statement?
  • Our billing information will appear as “Lockton Insurance Online.”
Won’t my employer cover me?
  • While your employer may cover you for your liabilities, it’s likely limited coverage that doesn’t provide full protection for your needs. With your own Professional Liability policy, you can ensure continuous protection at the level of coverage you require. Plus, personal coverage ensures you’re covered wherever you work.

What is a limit of liability?

  • The limit of liability on an insurance policy is the maximum dollar amount that the insurance company will pay if a claim is filed against you.

What limit of liability should I choose?

  • The limit of liability that you choose should correspond to the size and scope of your practice. As your professional skills grow and change, you should re-evaluate the limit of liability you carry.

    Available limits for this Professional Liability policy are:

    $500,000 per claim / $500,000 aggregate
    $1,000,000 per claim / $1,000,000 aggregate
    $1,000,000 per claim / $3,000,000 aggregate
    $2,000,000 per claim / $4,000,000 aggregate

    The occurrence limit is the maximum dollar amount for damages and expenses that your policy will pay for one claim. The aggregate limit is the maximum dollar amount your policy will pay in your defense per year.

Is this an occurrence or claims-made policy?

  • All our Professional Liability insurance policies are claims-made policies. Claims-made policies require the loss and claim to be filed within the active policy period for coverage to apply.If you are a nurse, you have the option of selecting a claims-made or occurrence policy. Occurrence policies require the loss to occur during the active policy period, but the claim can be filed after the effective dates of the policy.

HOW DO I MANAGE MY ACCOUNT?

Visit your “My Account” profile to manage your account and access your policy information.

You can also:

  • Request a policy cancellation
  • Request a change to the named insureds listed on your policy
  • Request an address change
  • Request a certificate of insurance
  • Update your contact information

WILL I GET A REMINDER TO RENEW MY CURRENT POLICY?

Yes, we will send your official renewal information via email from [email protected] when your policy is ready to renew.

HOW DO I REINSTATE MY POLICY IF IT HAS EXPIRED?

If your policy has been expired less than 30 days, please email us at [email protected] to request reinstatement.

If your policy has been expired for more than 30 days, you must reapply with our online application.

CAN I RENEW A GROUP PROFESSIONAL LIABILITY POLICY ONLINE?

Group coverage (which covers more than one person on the policy) cannot be renewed online at this time. We will send a paper renewal application about 90 days before your policy expiration date.

Filing a claim

HOW DO I REPORT A CLAIM?

At the first notice of a claim, lawsuit or potential claim, please contact your Lockton Affinity Health representative immediately. We will obtain the necessary information to start the claim process.

WHAT INFORMATION DO I NEED TO REPORT A CLAIM?

The more thorough the information you compile from the start, the more efficiently your claim will likely be settled.

Consider including:

  • Copies of incident reports, witness reports, photographs, videos and more
  • Your insurance policy number
  • Contact information for yourself and any claimants or involved parties

ONE OF MY PATIENTS WAS INJURED WHILE WITH ME BUT HAS NOT FILED A CLAIM. WHAT DO I DO?

If you think there is potential for an incident to turn into a claim, you can file an “incident only” report with us while the incident is still fresh in your mind. This ensures we have the information we need to help protect you and your business should the situation progress.
